A baby under 3 months with a temperature of 38°C (100.4°F) or higher needs a doctor the same day, and an older baby needs one if fever comes with poor feeding, unusual drowsiness, fast breathing, a rash or repeated vomiting.
Hospitals across Tamil Nadu added fever beds as dengue rose, with about 19,000 cases and nine deaths recorded up to late September 2026 (Tamil Nadu health registry / NCVBDC data). Chennai logged 1,293 dengue cases in September against 605 in August, and doctors urged parents not to ignore persistent or high fever in children or self-medicate. Rain-filled containers, coolers and tyres keep mosquitoes breeding through the northeast monsoon, so risk continues into winter. Having a trusted paediatrician consultation accessible without travel helps parents act quickly without panic.
You cannot reliably tell at home. This table shows why a doctor's assessment matters:
| Indicator | Common viral fever or cold | Possible dengue | Needs urgent care |
|---|---|---|---|
| Fever | Often 1 to 3 days, with runny nose or cough | High fever for 2 to 7 days | Any age under 3 months with 38°C+ |
| Baby's behaviour | Fussy but feeding and playful between spikes | Very irritable or lethargic, refuses feeds | Floppy, hard to wake |
| Other signs | Congestion, mild cough | Rash, vomiting, belly pain | Fast breathing, bleeding, fits |
With dengue, the dangerous phase often arrives around day 3 to 7, as the fever falls, so a baby who looks "better" but is more tired, vomiting repeatedly or passing very little urine still needs review. If you need newborn assistance at home, our baby care specialists provide hands-on monitoring.
| Level | What you see | What to do |
|---|---|---|
| Red | Under 3 months with 38°C+; floppy, very drowsy, blue, grey or pale; fit; rash that does not fade when pressed; fast, grunting or laboured breathing; bulging soft spot; any bleeding; no wet nappy in 8 hours | Go to emergency care or call 112 now |
| Amber | 3 to 6 months with 39°C+; fever beyond 2 to 3 days in dengue season (or beyond 5 days any time); feeding less than half; vomiting; wet cough with quick breaths | See a paediatrician the same day |
| Green | Fever under 2 days, feeding well, alert and playful | Home care, keep a log, recheck |
The under-3-months rule follows NHS paediatric fever guidance, which notes that fever in very young babies is more likely to signal serious infection. For overall newborn safety guidelines, check our newborn care in the first days guide.

Teething can make a baby fussy and slightly warm, but a true fever of 38°C or higher should not be blamed on teething.
Ask your paediatrician about annual influenza vaccination from 6 months, and keep routine immunisations up to date.
Generally after 24 hours without fever (without fever medicine) and with normal feeding and energy. Your paediatrician can confirm.
Not always. Your doctor decides based on days of fever, symptoms and local outbreak levels.
